About The Position

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist for a per diem opportunity completing home health visits with adult and senior clients in homes throughout the Flagstaff area. Prior home care experience is a plus but is not required. The Speech Language Pathologist is an integral member of a multidisciplinary health care team providing rehabilitative care and skilled nursing to clients so they can receive the medical care needed to remain at home.

Requirements

Current license as a Speech Language Pathologist in Arizona.
Possess a certificate of clinical competence in speech from the American Speech and Hearing Association; or meet the equivalent educational and work-experience requirements and be awaiting the opportunity to take the certification examination; or have completed the required education and work and be accumulating the supervised work experience required for certification.
Solid computer skills.

Nice To Haves

Prior home care experience as a Speech Language Pathologist.
Prior experience with electronic medical records (EMR).

Responsibilities

Make home visits to clients in designated geographic territories.
Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties and provide therapeutic interventions.
Develop treatment plans based on evaluations, physician’s orders, and recommended goals.
Continually assess and revise speech therapy care plans and participate with BAYADA clinicians in multidisciplinary care planning, as appropriate.
Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities that promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations related to patient care management and services provided using a touch pad tablet.
