About The Position

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist for a per diem opportunity completing home health visits with adult and senior clients in homes throughout its Sierra Vista geography. The Speech Language Pathologist is an integral member of a multidisciplinary health care team that provides rehabilitative care and skilled nursing, helping clients receive the medical care needed to remain at home. Prior home care experience is a plus but is not required.

Requirements

• Current license as a Speech Language Pathologist in Arizona.
• Meet one of the following certification or eligibility pathways: hold a Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association; have equivalent educational requirements and work experience and be awaiting the opportunity to take the ASHA certification examination; or have completed the necessary work and education requirements and be accumulating the supervised work experience required for certification.
• Solid computer skills.

Nice To Haves

• Prior home care experience as a Speech Language Pathologist.
• Prior experience with electronic medical records (EMR).

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate clients to assess and diagnose speech, language, communication, or swallowing difficulties, and provide therapeutic interventions.
• Develop treatment plans based on evaluations, physician orders, and recommended goals.
• Continually assess and revise speech therapy care plans, and participate in multidisciplinary care planning with BAYADA clinicians as appropriate.
• Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities that promote health, safety, and independent living.
• Accurately document observations, interventions, evaluations, patient care management, and services provided using a touch pad tablet.
