About The Position

BAYADA Home Health Care is seeking a Speech Language Pathologist for a per diem opportunity completing home health visits with adult and senior clients in homes throughout its Sierra Vista service area. The Speech Language Pathologist is an integral member of a multidisciplinary health care team providing rehabilitative care and skilled nursing so clients can receive the medical care needed to remain at home. Prior home care experience is a plus but is not required.

Requirements

Current license as a Speech Language Pathologist in Arizona.
Meet one of the stated certification pathways: hold a Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association; meet equivalent educational requirements and have the work experience necessary for the certificate while awaiting the opportunity to take the ASHA certification examination; or have completed the required work and education and be accumulating the supervised work experience required for certification.
Solid computer skills.

Nice To Haves

Prior home care experience as a Speech Language Pathologist.
Prior experience with electronic medical records (EMR).

Responsibilities

Make home visits to clients in designated geographic territories.
Evaluate clients to assess and diagnose speech, language, communication, or swallowing difficulties and provide therapeutic interventions.
Develop treatment plans based on evaluations, physician orders, and recommended goals.
Continually assess and revise speech therapy care plans and participate in multidisciplinary care planning with BAYADA clinicians, as appropriate.
Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities that promote health, safety, and independent living.
Accurately document patient-care observations, interventions, and evaluations using a touch pad tablet.
