About The Position

FOX Rehabilitation is hiring licensed Speech Language Pathologists to provide one-on-one care to older adults in Milwaukee, Wisconsin, and surrounding communities. Clinicians treat patients in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and support patient wellness, longevity, mobility, and functional independence. Clinicians work in a community-based, interdisciplinary practice alongside physical therapists, occupational therapists, speech language pathologists, and Fitness Specialists. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. Full-time and part-time roles are salaried, with benefits for eligible employees; PRN/FOX Flex roles are compensated per unit. Compensation varies by experience and employment type. Requirements

Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Speech Language Pathology program.
Basic computer and EMR proficiency.
Current CPR certification.
Current Certificate of Clinical Competence (ASHA CCC).
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, extended standing and walking, climbing stairs, and performing therapeutic activities.
Ability to wear PPE when required.
Ability to drive to patient homes and senior living communities.

Responsibilities

Provide one-on-one Speech Language Pathology services to older adults in private residences and senior living communities.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
Document effectively in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Create and manage a treatment schedule and build patient relationships.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
Potentially refer friends or patients, teach as a Clinical Instructor, mentor emerging professionals, provide care above bi-weekly quota, or create written and social media content for additional compensation.
