About The Position

21st Century Home Health Services is hiring a part-time Speech Language Pathologist for an in-person home health role serving a territory centered on Solano County, including Vallejo, American Canyon, and Hercules, with minimal visits to Napa County. The position requires fieldwork and visits to patients’ homes; remote and hybrid work are not available. Candidates must currently live in or plan to relocate to the territory. In partnership with the Case Manager, the Speech Language Pathologist provides skilled services to help patients with breathing, speaking, and swallowing functions. The clinician evaluates and treats speech, language, cognitive, and fluency disorders that affect communication and serves as an educator, patient advocate, and member of the interdisciplinary care team. The organization describes its model as combining home health care with structure, resources, and clinician support. It offers flexible scheduling, small coverage areas, autonomy, communication and software tools, and professional development. Candidates must complete a computer literacy and competency assessment before an in-person interview is scheduled. The assessment covers the ability to navigate electronic charting and the company’s web-based communication application, as well as understanding company values; results are confidential and used as one of several hiring decision inputs.
