About The Position

Ultimate Therapy is hiring a full-time Speech-Language Pathologist at Autumn Lake at Long Green in Baltimore, Maryland. The role supports skilled nursing residents with communication and swallowing challenges through treatment connected to their conversations, meals, and daily decisions.

The Speech-Language Pathologist evaluates speech, language, cognitive-communication, and swallowing function; develops individualized plans of care with measurable goals; and provides communication, cognitive, and dysphagia interventions. The position also includes educating residents, families, and caregivers, coordinating recommendations with nursing and dietary staff, documenting clinical findings, reassessing outcomes, and contributing to interdisciplinary care and discharge planning.

Requirements

• Active Maryland Speech-Language Pathologist license.
• Strong assessment and treatment skills in adult communication, cognition, and swallowing.
• Clear communication and accurate, timely clinical documentation.

• Skilled nursing or geriatric rehabilitation experience.

Responsibilities

• Evaluate speech, language, cognitive-communication, and swallowing function to determine appropriate services.
• Develop individualized plans of care with measurable goals that reflect residents’ abilities and priorities.
• Provide treatment for communication difficulties and functional challenges involving memory, attention, and problem-solving.
• Deliver dysphagia interventions and coordinate swallowing recommendations with nursing and dietary staff.
• Educate residents, families, and caregivers on strategies that support participation and carryover outside therapy.
• Document clinical findings, reassess outcomes, and contribute to interdisciplinary care and discharge planning.
