About The Position
Therapy Partner Solutions (TPS), a therapist-owned and operated company since 2016, is seeking a Speech Language Pathologist for home health in Flagler County. As the provider of home health therapy services for Accent Care Home Health, this role is part of a multidisciplinary team focused on coordinated, patient-centered care. TPS is dedicated to supporting its clinicians through mentorship, advanced training, and educational series, fostering an environment where therapists can thrive and grow their careers. The position emphasizes patient care and satisfaction, treating each patient with the utmost respect and care.
Requirements
- Graduate (current or pending) from an accredited SLP program.
- Active or pending state SLP license.
- New Graduates welcome to apply.
Responsibilities
- Provide exceptional therapy services in a home health setting.
- Work with a diverse and engaging caseload.
- Collaborate with a multidisciplinary team, including physical and occupational therapists, physicians, and other healthcare professionals.
- Deliver coordinated, patient-centered care.
- Participate in a comprehensive, team-based approach to care with AccentCare Home Health colleagues.
