Provides professional speech and audiology diagnostic and therapeutic services to clients as authorized by physicians and in compliance with state practice acts and licensure requirements. Serves as preceptor for new staff and provides functional supervision of HHAs. Prioritize and plan visits based on client condition, physician orders, and available resources. Maintain knowledge of laws, regulations, ethics, and agency policies. Assess clients at admission and required intervals using standardized documentation. Develop and revise treatment plans with physician and client, including attainable goals. Make referrals and coordinate care with other disciplines. Apply professional judgment in evaluations and interventions. Implement preventive, therapeutic, and rehabilitative procedures per treatment plan. Educate clients and families on communication and swallowing improvement. Report changes in client condition promptly and obtain updated orders. Participate in quality improvement and peer review. Document all care accurately and timely in the point-of-care system. Ensure safe home environment and discharge planning.
