About The Position

Human Touch Home Health is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our growing team serving patients throughout Falls Church, Arlington, Alexandria, McLean, Vienna, Annandale, Springfield, Fairfax, Reston, Herndon, and surrounding Northern Virginia communities. This is a PRN opportunity that offers flexibility, professional independence, and the ability to make a meaningful impact one patient at a time. As a Home Health Speech-Language Pathologist, you'll help patients improve communication, cognition, speech, language, and swallowing while restoring confidence and independence in the comfort of their own homes. You'll have the time to focus on each patient, celebrate progress, and provide care that truly changes lives. This role emphasizes personalized therapy, meaningful relationships, and lasting outcomes, distinct from crowded therapy departments or rushed appointments.

Requirements

Active Virginia Speech-Language Pathologist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills

Nice To Haves

Certificate of Clinical Competence (CCC-SLP) Preferred
Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus
Passion for Delivering Exceptional Patient-Centered Care

Responsibilities

Perform comprehensive speech, language, cognitive, and swallowing evaluations
Develop individualized treatment plans based on patient goals and clinical needs
Provide skilled speech therapy services in patients' homes
Educate patients and caregivers on communication strategies, cognitive techniques, and safe swallowing practices
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
