Requirements

Current state licensure as a Speech‑Language Pathologist
Master’s or Doctoral degree in Speech‑Language Pathology
Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting
Current CPR/BLS certification
Valid driver’s license and insured personal vehicle (per Lorian requirements)

Responsibilities

Evaluate patients’ speech, language, communication, and swallowing-related needs, including assessing the home environment and identifying appropriate equipment.
Develop, implement, and update individualized speech therapy treatment plans and Plans of Care (POC) in collaboration with the physician and care team.
Provide skilled speech therapy services using evidence-based therapeutic techniques and specialized equipment to improve functional outcomes and quality of life.
Monitor patient progress and communicate changes in condition, treatment response, and functional status with the physician, case manager, and interdisciplinary care team.
Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
Educate patients, caregivers, and other healthcare personnel on appropriate therapeutic strategies, communication techniques, and ongoing care needs.
