About The Position
Concept Rehab is a therapist-owned and operated company with over 45 years of experience providing high-quality care in Post-Acute / Skilled Nursing Facility (SNF) settings. They offer a supportive, growth-focused environment for clinicians. This is a part-time Speech Language Pathologist (SLP) position within a Post-Acute Rehab setting.
Requirements
- Master’s degree in Speech-Language Pathology
- Active state licensure and ASHA certification (new graduate/CFY candidates welcome to apply)
- Strong communication, clinical judgment, and patient-focused mindset
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapy for communication, cognition, and swallowing disorders
- Collaborate with physicians, care teams, and families
- Maintain accurate documentation, billing, and discharge planning
- Educate staff and caregivers on restorative programs
