About The Position

CenterWell Home Health is hiring a per diem Speech Language Pathologist to provide home health services to homebound patients who need speech services. Reporting to the Clinical Manager, the clinician delivers personalized rehabilitation and functional improvement services to help patients regain strength, mobility, independence, and confidence in everyday activities. The caseload is territory-based, and per diem scheduling is based on the clinician’s weekly availability.

Requirements

• Meet the education and experience requirements for ASHA Certification of Clinical Competence in Speech-Language Pathology or Audiology.
• At least six months of experience as a speech therapist or speech-language pathologist.
• Current, unrestricted license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct, and provide speech-language pathology services to patients in their homes or facilities.
• Participate in developing and periodically reviewing the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize their effects.
• Administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening.
• Observe, record, and report changes in patients’ conditions and treatment responses to a supervisor and/or physician.
• Instruct and train patients in the use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families, and healthcare staff.
• Maintain and submit required documentation.
• Prepare written evaluation, visit, summary, care plan, care coordination, and progress reports as required by company policy.
• Participate in care coordination and discharge planning.
• Protect confidential information and share it only when access is authorized and sharing is in the patient’s best interests.
