About The Position

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities throughout Fairfax County. This role offers flexibility in scheduling and territory selection, allowing therapists to build a caseload that works for them. The position focuses on providing one-on-one patient care in patients' homes, helping them improve communication, cognitive skills, and swallowing abilities. Requirements

• Active Virginia Speech-Language Pathologist License
• Graduate Degree in Speech-Language Pathology from an accredited program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to work independently in patients' homes

Responsibilities

• Complete speech-language pathology evaluations in patients' homes
• Develop individualized treatment plans
• Treat speech, language, communication, cognitive, and swallowing disorders
• Provide dysphagia treatment and swallowing education as appropriate
• Help patients improve functional communication and cognitive skills
• Educate patients, families, and caregivers
• Monitor progress and adjust treatment plans as needed
• Coordinate care with the interdisciplinary team
• Complete reassessments and discharge planning
• Maintain accurate and timely documentation using WellSky
