About The Position

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities throughout Prince William County. This role offers flexibility in scheduling and territory, allowing therapists to choose the communities they want to cover and build a caseload that works for them. The position focuses on helping patients improve their communication, cognitive, and swallowing abilities in the comfort of their own homes.

Requirements

• Active Virginia Speech-Language Pathologist License
• Graduate Degree in Speech-Language Pathology from an accredited program
• Current ASHA Certification (CCC-SLP)
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to work independently in patients' homes

Responsibilities

• Complete speech-language pathology evaluations in patients' homes
• Develop individualized plans of care
• Treat speech, language, communication, cognitive, and swallowing disorders
• Provide dysphagia treatment and swallowing education as appropriate
• Help patients improve functional communication and cognitive skills
• Educate patients, families, and caregivers
• Monitor progress and adjust treatment plans as needed
• Coordinate care with the interdisciplinary team
• Complete reassessments and discharge planning
• Maintain accurate and timely documentation using WellSky
