About The Position

Concept Rehab is seeking a compassionate and motivated full-time Speech-Language Pathologist to join its rehabilitation team in Okemos, Michigan, at a skilled nursing facility. The therapist helps patients improve communication, swallowing, and quality of life by evaluating their needs, providing individualized treatment, monitoring progress, and collaborating with patients, families, caregivers, and other healthcare professionals. Concept Rehab is therapist-owned and operated and has over 45 years of experience providing rehabilitation services.

Requirements

Master's degree in Speech-Language Pathology from an accredited program.
Current Michigan Speech-Language Pathologist license or eligibility to obtain licensure.
Strong communication, organizational, and clinical documentation skills.
Compassionate, patient-centered approach to care.

Nice To Haves

Certificate of Clinical Competence (CCC-SLP) preferred; eligible CF candidates may be considered.
Experience in a skilled nursing facility or post-acute rehabilitation setting preferred.

Responsibilities

Evaluate patients and develop individualized speech therapy treatment plans.
Provide speech-language therapy to patients with communication, cognitive, and swallowing disorders.
Assess and treat dysphagia and other speech-related conditions as appropriate.
Monitor patient progress and adjust treatment plans to individual needs.
Complete accurate and timely clinical documentation in accordance with facility and regulatory requirements.
Collaborate with physicians, nursing staff, occupational therapists, physical therapists, and other healthcare professionals.
Educate patients, families, and caregivers on treatment plans and strategies.
Participate in care planning and interdisciplinary team meetings as needed.
