Speech Language Pathologist (SLP) - Per-Diem
About The Position
Centers Home Therapy (CHT) is expanding its footprint and we are seeking Speech Therapist to provide home-based speech therapy services. Responsibilities: Make home visits to clients. Evaluates patients referred for home care services to determine appropriateness for speech therapy services. Establishes a home care plan including assessment of the appropriateness of the requested services. Provides rehabilitative services for speech and language disorders to meet established treatment goals. Evaluate patients’ conditions for swallowing or cognitive deficits. Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression. Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers. Keep accurate records of patient’s progress, goals and attendance. Selects and use appropriate diagnostic and therapeutic techniques and materials. Documents type of treatment and patient’s response on designated clinical progress notes.
Requirements
- Master's degree in Speech Therapy or a foreign degree equivalent as accepted by US Citizenship and Immigration Services or a Bachelor's Degree or foreign degree equivalent plus 2 years of progressive, post-baccalaureate experience.
- Valid New York State license as a Speech Language Pathologist.
- Excellent communication skills and the ability to interact well with diverse individuals.
- Basic computer skills.
- New graduates welcome
- Able to function independently, with minimum supervision
- Current Driver’s License, required insurance and car available for work
