About The Position

Responsible for outreach in the community to providers and stakeholders to increase referrals of children and adolescents eligible for services funded by the State Opioid Response (SOR) Grant. Complete screening, assessments and Intakes of eligible members. Act as the primary point of contact for all members, activities and services funded by the SOR Grant. Work with youth/families involved in the juvenile justice system. Facilitate meetings and follow up on recommendations, to ensure continuity of care and service delivery, and educate members about service options and delivery. Must be able to obtain an IVP Level 1 Fingerprint Clearance Card and maintain clearance throughout employment.

Requirements

  • Bachelors or Master's Degree, no experience necessary.
  • Associate's degree plus two (2) years Behavioral Health work experience.
  • High School Diploma or GED plus four (4) years of full-time Behavioral Health work experience.
  • Level 1 IVP Fingerprint Clearance Card
  • Must be 21 years of age (insurance requirement)
  • Possess a valid AZ driver’s license with a “clean” or “acceptable” driving record.
  • An unacceptable driving record includes (but is not limited to) the following: more than two [2] moving violations in the last three [3] years or; more than one [1] at-fault accident within the last three [3] years or; any major violation (i.e. DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years. Other driving related violations may be considered unacceptable.
  • Must be able to pass a pre-employment drug screen and criminal background check.
