About The Position

AccentCare is hiring a full-time Hospice Social Worker / MSW in Clearwater, Florida, covering Clearwater and surrounding areas. The position is not remote or virtual. The social worker supports patients and families receiving pre-hospice and hospice care, helping the care team understand the social, spiritual, and emotional factors affecting each patient and contributing to a plan of care that promotes personal worth, spiritual well-being, and human dignity. The role participates in the interdisciplinary team, helps patients and families navigate illness-related changes, connects them with available resources, and supports the site bereavement program.

The posting describes the position as a Hospice Social Work Fellow and requires a BSW from a Council on Social Work Education-accredited school, with an MSW preferred, completion of an approved BSW- or MSW-level hospice internship, applicable state-specific qualifications, and at least one year of experience in a health care setting. Medicare, Medicaid, or insurance specialty experience is preferred.

Requirements

• Bachelor of Social Work (BSW) degree from a school of social work accredited by the Council on Social Work Education; a Master of Social Work (MSW) is preferred.
• Successful completion of an approved undergraduate-level BSW or graduate-level MSW hospice internship.
• Meet applicable state-specific requirements above minimum education standards.
• At least one year of experience in a health care setting.

Responsibilities

• Help the pre-hospice and hospice care team understand social, spiritual, and emotional factors related to patients’ health and contribute to plans of care that foster personal worth, spiritual well-being, and human dignity.
• Participate in the interdisciplinary team and in the development and review of plans of care for assigned patients.
• Prepare patients to deal with changes related to illness and help families support patients, including through education about advance directives.
• Use available resources, including family, hospice, and community agencies, to help patients and families live better within the limitations of illness.
• Support the site-level bereavement program.
