About The Position

Haven Hospice is seeking a dedicated Social Worker to serve patients in Palatka, Florida, and the surrounding area. Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness, helping ensure their psychosocial needs are met with dignity and respect. The position supports holistic, person-centered hospice care and involves working independently while receiving team support.

The role is based at 400 St. Johns Avenue, Palatka, FL 32177, with coverage of Palatka and surrounding areas. The schedule is Monday through Friday, 8 a.m. to 5 p.m. The position is subject to a Level 2 background screening.

Requirements

• Graduate of a Council on Social Work Education-accredited master’s program in social work (MSW) or bachelor’s program in social work.
• Meet personnel qualifications stated in §418.114(b)(3).
• At least one year of experience in health care.
• Understand hospice philosophy and issues related to death and dying.
• Demonstrate good verbal and written communication and organizational skills.
• Hold current licensure, according to Medicare guidelines, to practice in the state of operation.
• Be a licensed driver and have an insured, properly maintained automobile meeting state or organizational requirements.

Responsibilities

• Assess the psychosocial needs of patients and families.
• Evaluate home safety and assist with alternative care plans when needed.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate patients and families about advance directives and assist with them.
• Provide referrals and practical support for patients and families.
• Serve as a liaison with community resources and agencies.
• Collaborate with team members to support holistic patient care.
• Support families with funeral, financial, and legal planning.
