About The Position

The Social Worker is responsible for resident assessment, care management, care planning, addressing psychosocial concerns, and may serve on the Behavior, Pre-Care Plan, and Care Plan committees.

Requirements

• Ability to give instructions, organize time and complete tasks with minimal supervision.
• Ability to manage multiple requests and projects simultaneously.
• Excellent written, verbal and interpersonal skill necessary.
• Masters in Social Work required

Nice To Haves

• A broad knowledge of social work skills and the ability to apply necessary skills and techniques in a medical setting is preferred.
• Long-term care social work experience preferred.
• Experience working with PointClickCare software a plus.

Responsibilities

• Responsible for exercising discretion and independent judgment to assist the management of the Social Services Department
• Will encourage residents through counseling and other interventions to achieve their highest level of psychosocial interaction
• Will implement person-centered care plans for residents and will monitor and follow-up on directives as outlined in the resident's plan of care
• Counsel residents/families regarding psychosocial, adjustment, and behavioral issues.
• Participate in Behavior Management Team meetings to monitor resident behaviors and psychotropic medication use
• Participate in interdisciplinary care conferences with residents and families
• Assist with resident discharge planning
• Performs other duties as required.
