Reserve Care (formerly Hospice of the Western Reserve) is seeking a full-time Licensed Social Worker to join their team. Reserve Care provides expert hospice, palliative, and grief support. The Social Worker will be pivotal in providing counseling services and medical social work to terminally ill persons and families in collaboration with the transdisciplinary team in a variety of settings. The role involves addressing emotional, spiritual, and psychosocial needs, assisting with support programs, maintaining records, participating in care conferences and bereavement programs, and assisting with discharge planning and referrals to community resources. Flexibility is required for emergency situations, working evenings and weekends, and participating in on-call rotations. The role also involves financial assessments, maintaining transmission-based precautions, participating in quality improvement activities, coordinating with volunteers, completing competencies, assisting with Medicaid applications, assigning new cases, orienting new employees, assisting with in-service programs and public relations, and participating in committees and case reviews. The position may also involve supervising Social Work Interns.
