About The Position

Social Work Services at The Mount Sinai Hospital provides comprehensive social work services and programming to meet the needs of diverse populations. The department emphasizes the physical, emotional, and social needs of each patient and their care partners, with social workers collaborating within interprofessional teams on both micro and macro levels. The Department of Social Work Services is one of the oldest in the nation, offering a pathway to LCSW licensure and access to continuing education and professional development. This specific role is within the Center for Transgender Medicine & Surgery (CTMS), utilizing a multidisciplinary team approach to provide coordinated care to trans-identified patients seeking gender-affirming medical interventions, including surgeries. The position is based at Mount Sinai’s Downtown Practice.

Requirements

  • Experience working with LGBTQIA+ individuals with specific familiarity with transgender individuals.

  • A collaborative and flexible attitude to meet the changing needs of the interdisciplinary team.
  • NYS LMSW or LCSW

Responsibilities

  • Complete full psychosocial assessments.
  • Work closely with the CTMS interdisciplinary team (nurse coordinator, eligibility assistant, mental health professionals, primary care providers, surgeons, medical specialists, outside service providers) to support trans patients through their medical transitions.
  • Facilitate appropriate medical interventions when required.
  • Confirm patients have complete mental health evaluations, all necessary letters, and pre-surgical treatments.
  • Determine that post-surgical resources for care in the community are in place, collaborating with inpatient social workers, discharge planners, and outside agencies.
  • Develop and maintain support groups for CTMS patients (pre- and post-surgical).
  • Provide short-term therapy (pre- and post-op).
  • Participate in CTMS team meetings, steering committee meetings, and community advisory board meetings.
  • Create and organize patient lists with up-to-date surgical information.
  • Co-lead case conference meetings to discuss high-risk patient issues.
  • Mediate between in-patient and out-patient teams to facilitate smooth and comprehensive care throughout the surgical process.
