About The Position
The CSWA Social Worker provides comprehensive biopsychosocial and spiritual assessment, diagnosis, and/or treatment, interventions, and advocacy for patients and their support systems under direct and continuous supervision. The role applies concepts and theories related to life-span development, neurobiology, and behavioral change. The Social Worker collaborates with patients, their support systems, and the Inter/Multi-Disciplinary Team to develop and implement evidence-informed, person-centered care plans across the care continuum. Duties are performed in a manner that respects patient self-determination, reduces health disparities and barriers to care, and follows the NASW Code of Ethics and Standards of Practice. This is an on-call, day-shift position at Providence Milwaukie Hospital.
Requirements
- Master's degree in Social Work.
- Obtain an Oregon Clinical Social Work Associate License within 60 days of hire.
Nice To Haves
- Two years of clinical work experience in healthcare, hospice, and/or bereavement.
Responsibilities
