•$88,690 - $101,671•Onsite

About The Position

The Social Worker will provide services to adult patients diagnosed in childhood with inborn errors of metabolism, conditions that require lifelong management and treatment with specialized diets and medications. As advances in therapy and treatment enable more patients to live into adulthood, the social worker will support patients with both chronic conditions and disorders that may involve catastrophic crises. This 0.6 FTE position works Monday, Tuesday, and Thursday, primarily in the outpatient Metabolic clinic. As an active member of the interdisciplinary medical team, the social worker provides short- and long-term individual and family care, including psychotherapy, care management, crisis intervention, health education, and advocacy as patients’ medical and developmental needs evolve. The role may include end-of-life and bereavement care and support for patients and families throughout that process. The position is based in person. The social worker may liaise with adult inpatient social workers to support continuity during hospitalizations, including addressing metabolic-specific resource needs involving medications and formulas. Limited cross-coverage with the Pediatric Metabolic Social Worker is provided during PTO.
