Mount Sinai Health Systems•New York, NY
•$88,690 - $101,671•Onsite

About The Position

The Social Worker will provide services to adult patients diagnosed with an inborn error of metabolism during childhood. These conditions require lifelong management and treatment with specialized diets and medications, and patients may experience either catastrophic crises or chronic illness. The role provides short- and long-term individual and family care through psychotherapy, care management, crisis intervention, and health education as an active member of an interdisciplinary medical team. Patients are followed primarily in the outpatient clinic. The role may include end-of-life and bereavement care, requiring support for patients and families through those experiences. Advocacy in response to patients’ evolving medical and developmental needs is central to the work. The position is based in person in the outpatient Metabolic clinic. The Social Worker will sometimes coordinate with adult inpatient Social Workers to support continuity during hospitalization, including assistance with metabolic-specific resources, medications, and formulas. Limited cross-coverage with the Pediatric Metabolic Social Worker is expected during PTO. This is a 0.6 FTE position with Monday, Tuesday, and Thursday workdays.
