About The Position

This full-time Social Worker position is responsible for participating in planning related to resident, family, and community interests. The role involves discharge planning, development and implementation of social care plans, and resident assessments. Administrative duties include completing medical forms, reports, evaluations, studies, charting, and coordinating activities with other departments. The Social Worker promotes optimum transition for resident discharge, maintains ongoing communication with residents and their responsible persons regarding progress, and provides information on financial assistance programs like Medicare/Medicaid. Additionally, the role offers consultation to staff and community agencies to develop social service programs and solve resident concerns.

Requirements

  • Must possess a bachelor's or master's degree in social work or a bachelor's or master's degree in a human services field (i.e. sociology, special education, rehabilitation counseling and psychology.)
  • One year of supervised social work experience in a healthcare setting working directly with individuals.
  • Experience in long-term care, working with the senior population and dementia counseling.
  • Must be licensed as a social worker in Michigan.
  • Must lead by example and possess superior customer service skills and professionalism.
  • Flexibility to adapt to ongoing change and work in a fast-paced, customer driven environment.

Responsibilities

  • Participate in planning related to resident, family, and community interests.
  • Participate in discharge planning, development and implementation of social care plans and resident assessments.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, arranging transportation, etc., as necessary, coordinating activities with other departments as necessary.
  • Promotes optimum transition of resident for appropriate and safe placement for discharge.
  • Provides ongoing communication with resident and responsible persons of progress/status during stay.
  • Provide information to the resident/family on Medicare/Medicaid as well as other financial assistance programs available.
  • Provide consultation to staff, community agencies, etc., in efforts to solve resident concerns through the development of social service programs.
