Social Worker for Hospice
About The Position
The PRN Hospice Social Worker provides flexible coverage and support as patient and organizational needs arise. The role is designed to assist with weekend coverage, including respite visits, urgent psychosocial needs, and support for patients and families requiring additional services. The Social Worker also provides coverage during team member vacations, leaves of absence, and periods of census growth to help ensure continuity of care. Caseloads and visit expectations vary based on organizational need, patient acuity, geographic location, and coverage requirements. Compensation is per-visit or hourly, based on licensure and experience, ranging from $30 to $39.90 per hour. Additional compensation may be available for weekend and on-call responsibilities, when applicable.
Requirements
- MSW, LSW, or LCSW.

Responsibilities
- Conduct psychosocial assessments and routine social work visits.
- Support patients and families with resource coordination, counseling, and end-of-life planning.
- Participate in care conferences and interdisciplinary collaboration as needed.
- Provide weekend respite and supportive visits.
- Assist with caseload coverage during vacations, leaves of absence, and census fluctuations.
- Complete timely documentation in accordance with agency standards.
