Social Worker for Hospice
About The Position
The PRN Hospice Social Worker provides coverage and support as patient and organizational needs arise. This role is designed to assist with weekend coverage, including respite visits, urgent psychosocial needs, and support for patients and families requiring additional services. The social worker may also provide coverage during team member vacations, leaves of absence, and periods of census growth to help ensure continuity of care. Caseloads and visit expectations vary based on organizational need, patient acuity, geographic location, and coverage requirements. Compensation is per visit or hourly, ranging from $30 to $39.90 per hour based on licensure and experience. Additional compensation may be available for weekend and on-call responsibilities, when applicable. This opportunity offers flexibility for experienced hospice professionals or social workers looking to supplement an existing practice or part-time schedule while remaining connected to a collaborative hospice team providing patient- and family-centered care.
Requirements
- A qualifying social work credential: MSW, LSW, or LCSW.

Responsibilities
- Conduct psychosocial assessments and routine social work visits.
- Support patients and families with resource coordination, counseling, and end-of-life planning.
- Participate in care conferences and interdisciplinary collaboration as needed.
- Provide weekend respite and supportive visits.
- Assist with caseload coverage during vacations, absences, and census fluctuations.
- Complete timely documentation in accordance with agency standards.
