Requirements

  • Bachelor's Degree from an accredited school of Social Work (Required)
  • Licensed Social Worker (LSW) (Required)
  • Computer skills (Required proficiency)
  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
  • Multi-tasks and prioritizes work (Required proficiency)

Nice To Haves

  • Master’s Degree from an accredited school of Social Work (Preferred)
  • Hospital/Health Agency experience. (Preferred)
  • Communicates effectively with persons of various backgrounds (Preferred proficiency)
  • At least 2 years of recent experience is strongly preferred.
  • Previous hospital experience is also preferred.

Responsibilities

  • Provides psychosocial assessment / intervention to assist patients/families.
  • Provides age specific care to patients/families in all age groups.
  • Develops and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
  • Develops plans with attention to individual patient and family goals
  • Provides linkage to community resources to facilitate optimal care
  • Communicates/collaborates with the work team using a multidisciplinary approach
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
