About The Position

This is an exciting opportunity to work within a collaborative multidisciplinary team including physicians, nurses, case managers, and rehab staff to provide quality care to patients and families on the inpatient hematology oncology service. The Social Worker provides a key role in assessing and supporting the psycho-social needs of patients and their support networks and addressing barriers to care to help optimize outcomes. Opportunity to utilize brief counseling skills to support patient adjustment and engagement in treatment. Collaborate with interdisciplinary care team and community-based agencies to help address barriers and facilitate transitions along the continuum of care. Clinical supervision and professional development are provided and highly valued.

Requirements

  • Master's degree in Social Work
  • Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint, OneDrive and SharePoint.

Nice To Haves

  • Experience in Oncology or hospital Social Work preferred.
  • Relevant health care and/or human service experience.

Responsibilities

  • Provides clinical services to patients and families that address personal and environmental issues in order to maximize emotional, social, and physical well-being and the appropriate and efficient use of health care resources.
  • Screens and triages patients who request service, are referred, or meet screening criteria for identifying patients' needs.
  • Conducts psychosocial assessments and provides services as indicated.
  • Partners with the multidisciplinary team to facilitate continuity of care planning.
  • Meets productivity standards and expectations as set by the leadership in Social Work and the assigned clinical area.
  • Provides counseling and psychotherapeutic services appropriate to setting.
  • Provides timely and appropriate communication and documentation in the medical record.
  • Provides consultation and education regarding psycho-social issues affecting patient care.
  • Advises and addresses ethical and risk management issues.
  • Presents and/or discusses clinical work in formal and informal settings, multi-disciplinary and clinical rounds and seminars.
  • Participates in department or hospital process improvement and CQI efforts as needed.
  • Partners with advanced staff on research.
  • May present clinical material and theory at teaching conferences and seminars.
  • Staff licensed as an LICSW may supervise staff or student interns.
  • Serves as a content resource for multidisciplinary staff; and mentor/trainer to less experienced staff.
  • Encourages an environment of idea sharing and feedback.
  • Seeks opportunities and implements goals for own professional development.
