About The Position

The Case Manager Social Worker assists patients with complex psychosocial needs, including determining eligibility for social programs and funding sources and qualifying for community assistance from special funds and agencies. The role provides short-term counseling and crisis intervention to patients and families with psychosocial needs, and coordinates and facilitates safe discharge plans of care for high-risk patient populations.

Working with the RN Case Manager, the Social Worker manages socially complex patients identified through initial assessments and referrals. The Social Worker identifies resources to meet patients’ and caregivers’ ongoing needs across the care continuum and is required to initiate this plan on admission.

Requirements

• Graduate from an accredited Social Work Program with a Bachelor of Social Work.
• Clinical rotation experience may be considered in lieu of experience if applicable to the job.

Nice To Haves

• Master of Social Work degree.
• Experience in hospital social work.
• Knowledge of managed care practices, community resources, regulatory issues, and insurance reimbursement issues.
