About The Position

The Social Worker coordinates or performs administrative functions related to admissions and discharges of residents in Healthcare and assists with organizing and planning Social Services for community residents. The role works with Healthcare Team Members and outside providers to facilitate admissions, complete required documentation, maintain resident records, and coordinate financial updates for private-pay residents. The position also completes the appropriate section of the MDS and participates in scheduled in-service training and education.

Requirements

High school graduation or GED.
Bachelor’s degree in Social Work or a closely related human services field.
One to two years of related experience in a healthcare setting.
Two years of experience in a similar social work position.
Ability to read, write, speak, and understand English fluently.
Ability to meet or exceed company attendance and punctuality standards.
Ability to use electronic records, miscellaneous software, and office equipment.
Ability to understand and follow directions.
Ability to work with minimal supervision.

Nice To Haves

Two years of long-term care or rehabilitation experience desired.

Responsibilities

Assist with responsibilities and functions delineated in the discharge plan.
Coordinate with Healthcare Team Members to complete administrative and clerical functions related to resident admissions and discharges.
Maintain appropriate contact with hospitals, doctors, and outside agencies to facilitate admissions.
Coordinate all forms needed for admissions.
Maintain the state-required resident roster.
Coordinate financial updates for private-pay residents.
Complete the appropriate section of the MDS.
Attend and participate in scheduled in-service training and education programs.
Perform other duties as assigned.
