The Social Worker (PRN) will perform a variety of duties to address the social, psychological and economic needs of children and their families, in both an inpatient and outpatient setting. The person in this role is concerned with continuity of care which usually involves discharge planning for children who are inpatients and their families to ensure they understand and follow the recommended treatment plan, and referrals to community agencies and programs.

Requirements

1+ years of previous social work or related field experience required
Master's Degree MSW required
Wisconsin Advanced Practice Social Work (APSW) certification required
One of the following certifications/licenses: License Social Worker Advance Practice (121) - State of Wisconsin, License Social Worker Clinical (123) - State of Wisconsin, License Social Worker Independent (122) - State of Wisconsin

Responsibilities

Perform a variety of duties to address the social, psychological and economic needs of children and their families, in both an inpatient and outpatient setting.
Address continuity of care, which usually involves discharge planning for children who are inpatients and their families to ensure they understand and follow the recommended treatment plan.
Make referrals to community agencies and programs.
Understand and satisfy the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent and/or adult patient in regards to their growth and development process.
Assist families in meeting their needs through short term, crisis oriented counseling.
Educate patients/families about illness, hospital procedures and community resources.
Help plan for the future health care needs of the child whether or not admitted to the hospital.
Consult with other professional staff to assure that they understand the strengths and needs of patients and families.
Assess the medical care/history of the child and family to assist medical staff with diagnosis and treatment plan.
Perform duties in a courteous and helpful manner with respect towards patients/families, department staff, visitors and employees from other departments.
