The Graduate Social Worker is responsible for providing effective and appropriate social, physical, educational, and therapeutic services to meet the needs and interests of residents throughout the community. This role involves completing resident assessments, planning and implementing programs, evaluating resident behaviors, conducting one-on-one visits, attending Care Plan meetings, and maintaining documentation. The Social Worker will also be involved in coordinating admissions, assessing incoming residents, developing intervention techniques for behavioral concerns, advocating for residents, assisting with transfers, and maintaining contact with families. Additionally, the role includes coordinating discharge planning, informing residents and responsible parties about healthcare choices, arranging outside support services, offering counseling, advocating for resident rights, and participating in staff education. The Social Worker may also assist the Lifestyle Department and instruct volunteers. A key aspect of the role is to improve the quality of care and quality of life for residents across all continuums of care, while adhering to safety codes and infection control procedures.
