Partners Community Health (PCH) is a new not-for-profit organization focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of-the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and will be part of PCH’s larger strategy to introduce innovative and inclusive programs and services and new models of care delivery. PCH is committed to building partnerships and connections that put people first, as well as a learning and leading healthcare community that provides best-in-class service across the aging continuum. PCH is seeking a Social Worker to provide resident-centred social work services in accordance with the Fixing Long-Term Care Act, 2021, Ontario Regulation 246/22, the Social Work and Social Service Work Act, 1998, applicable professional standards, and PCH policies. As a member of the interdisciplinary team, the Social Worker assesses and addresses residents’ psychosocial needs, supports residents and lawful decision-makers, and promotes dignity, autonomy, safety, and quality of life. The Social Worker is accountable for competent, ethical, resident-centred practice; informed consent; confidentiality; accurate records; professional registration; and compliance with applicable law, professional standards, and PCH policies.
