About The Position
The Social Worker engages patients and families in identifying psychosocial needs to develop appropriate interventions and coordinate optimal, safe plans of care. The role provides education, counseling, and referrals as indicated, and plans and communicates effectively with the multidisciplinary team and community resources. The position supports the Information Sharing Core Element of PFCC.
Requirements
- Master's degree in social work from an accredited program.
- New Jersey Social Work License required; MSW and LSW or LCSW required.
- Effective group process skills.
- Effective oral and written communication skills to express ideas clearly.
- Effective listening skills.
- Ability to attend to details and switch between tasks without losing efficiency or composure.
- Ability to follow through using appropriate communication channels.
- Ability to function competently in stressful situations and changing work environments related to changing patient needs, including work with patients with acute, chronic, and complex disease processes.
- Ability to work cooperatively with patients, family members, and multidisciplinary team members.
- Ability to use effective time management to set priorities, perform job responsibilities, and respond quickly to emergency situations.
- Ability to use analytical and critical-thinking skills to solve problems.
