About The Position

Works as a member of the health care team, following social work practice and professional standards, to facilitate continuity of care. Assesses patients’ psychosocial needs and plans and coordinates discharge follow-up and care appropriate to each patient’s level of care. Provides supportive counseling and crisis intervention as needed. Provides patients and their families with resources related to housing and placement, legal issues, and referrals.

Requirements

• Master’s degree in Social Work (MSW).
• Current licensure in the Commonwealth of Pennsylvania as a Social Worker, or eligibility for licensure; license must be obtained within six months of hire.
• Strong verbal and written communication skills.
• Ability to work independently and gather pertinent patient information.
• Fundamental understanding of personal computers and familiarity with the Windows operating system.
• Strong negotiation skills and ability to address complex issues with patients, families, and staff across multiple departments.

• One year of psychiatric inpatient experience in a social work position.
