About The Position

The Social Worker serves as a liaison between the hospital and community agencies or facilities, exchanging clinical and referral information. The role identifies patients with psychosocial or financial risk, assesses the psychological needs of patients and families, and provides information, support, counseling, care management, and referrals to appropriate resources. The Social Worker works with the interdisciplinary team to coordinate care and meet patient needs across the continuum, resolve psychosocial care issues, and develop and implement complex patient transition and discharge plans. The position also supports participant screening and evaluation of psychosocial surveys, helps participants develop skills for integrating lifestyle changes into daily life, and contributes to care planning and monitoring. Work is typically performed in a clinical environment. This is a full-time, day-shift position at Geisinger Medical Center, with hours of 8:00 a.m.–4:30 p.m. or 8:30 a.m.–5:00 p.m., Monday through Friday, plus a weekend rotation.
