About The Position

Requirements

Master’s Degree in Social Work
Current registration with the Ontario College of Social Workers
Training and educated in CBT and/or DBT modalities, and Mindfulness
Experience working with individuals with complex mental health and addiction issues, and trauma
Experience in program development and delivery
Ability to work independently and within an interdisciplinary team
Good communication and organizational skills
Effective computer skills and record keeping

Nice To Haves

Experience with family therapy and/or motivational interviewing preferred

Responsibilities

Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability to make effective use of treatment
Integrate the biopsychosocial history into the treatment plan
Develop and evaluate treatment plans in collaboration with the clinical team, community supports and families
Facilitate group counselling sessions
Provide individual counselling in support of patient’s specific needs and goals
Ensure continuity of care is maintained through the transfer of information
Work with the clinical team to initiate community treatment orders
Establish relationships with key community supports
Discharge planning
Prepare referrals to appropriate community services
Document in a timely manner
Collaborate with the multidisciplinary team
