About The Position

Lake Cumberland Regional Hospital is seeking an Acute Social Worker for its Pediatric, Nursery, Mother/Baby, and Labor and Delivery units. This is a full-time, day shift position. The role involves assessing and planning interventions to help patients cope with social, emotional, economic, and environmental problems. The social worker will interview patients, families, and caregivers to determine and arrange for needed services, participate in case reviews, consult with healthcare team members, and assist with discharge planning. This includes identifying and referring patients to appropriate resources such as transportation, housing, healthcare, and social/spiritual services. The position also involves supervising and/or training new staff, students, and interns, and providing individual, family, and group therapy.

Requirements

  • Bachelors required; Masters preferred
  • Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire
  • Handle With Care within 30 days of hire.
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Nice To Haves

  • Experience working with patients with SUD/OUD, child protective services, and perinatal mood disorders.

Responsibilities

  • Assesses and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Assists with discharge planning and processes.
  • Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Supervises and/or trains new staff, students, and interns.
  • Provides individual, family and group therapy.
