This individual will be responsible for providing medical social services to the patient and family in accordance with the physician's plan of care within the guidance of State and Federal Regulations. The role of the SRMC Social Worker is to evaluate patients, determine their needs, and formulate an action plan to ensure that those needs are met efficiently. The SRMC Social Worker will work with all customers throughout the Pole Creek Campus that require assistance.

Requirements

  • Bachelor’s degree (BSW) in Social Work, Masters (MSW) preferred.
  • Prefer at least 2 years of experience in the medical social work field (acute care and geriatrics) or community social work.
  • Must have a current and valid license as a Social Worker in the State of Nebraska.

Responsibilities

  • Provide Medical Social Services to the patients/ residents and their families in accordance with the physician’s with the guidance of State and Federal Regulations.
  • Evaluate patients, determine their needs, and formulate an action plant to ensure that those needs are met efficiently.
  • Work with all customers throughout the Pole Creek Campus that require assistance.
  • Provides Resident/Staff care and Social and Spiritual Needs
  • Performs and maintains documentation.
  • Communicate effectively.
  • Promote and ensures patient safety in performance of all responsibilities.
  • Assist/ implements Admit/Discharge planning services, human resources and financial goals and objectives consistent with Acute Care’s goals and objectives.
  • Follow HIPAA regulations and ensures that the confidentiality of patients’ medical, personal, financial records, employee records, and organizational records are maintained.
  • Comply with state and federal regulatory requirements related to the performance of operations and requirements of the company Compliance Program.
  • Participate in quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations.
  • Present self in a professional manner and enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops.
  • Other duties as assigned by management.
