About The Position

The Social Worker is responsible for providing social work services to residents and their families within our Skilled Nursing Facility (SNF). This role involves assessing psychosocial needs, coordinating care, and advocating for residents to enhance their overall well-being.

Requirements

  • Bachelor's or Master's degree in Social Work (MSW) from an accredited program.
  • State licensure or certification as a Licensed Clinical Social Worker (LCSW) is required.
  • Previous experience in a social work role, preferably in a healthcare or long-term care setting.
  • Strong assessment and counseling skills.
  • Empathy and sensitivity in working with diverse populations, including elderly individuals and their families.
  • Effective communication and interpersonal skills.
  • Knowledge of relevant laws, regulations, and ethical standards in social work.
  • Ability to collaborate with interdisciplinary teams and community agencies.
  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with customers, personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Must be able to move intermittently throughout the workday.
  • Must be able to read, speak, and understandably write the English language.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with customers, residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this company, which include a medical and physical examination if the position requires.
  • Must be able to push, pull, move, and/or lift a minimum of 50 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

Responsibilities

  • Conduct comprehensive psychosocial assessments for residents to identify social, emotional, and environmental factors impacting their well-being.
  • Collaborate with interdisciplinary teams to integrate psychosocial information into individualized care plans.
  • Provide counseling and emotional support to residents and their families, addressing issues such as adjustment to long-term care, grief, and end-of-life concerns.
  • Facilitate support groups and educational sessions as needed.
  • Coordinate and collaborate with healthcare professionals, including nurses, therapists, and physicians, to ensure holistic care for residents.
  • Advocate for residents' needs within the care team.
  • Develop and implement discharge plans for residents transitioning from the facility to other care settings or their homes.
  • Coordinate with post-discharge service providers and community resources.
  • Assist residents and their families in developing advance care directives and making informed decisions about medical treatment.
  • Facilitate discussions about end-of-life preferences.
  • Connect residents and families with community resources and support services, including financial assistance, legal aid, and transportation services.
  • Stay informed about available resources and maintain a comprehensive referral network.
  • Communicate regularly with residents' families to provide updates, address concerns, and involve them in care planning.
  • Conduct family meetings as needed.
  • Provide crisis intervention and support during emergencies, sudden health declines, or other challenging situations.
  • Collaborate with the care team to ensure a coordinated response.
  • Maintain accurate and up-to-date documentation of social work assessments, interventions, and interactions with residents and families.
  • Ensure compliance with regulatory standards and facility policies.
  • Advocate for residents' rights, dignity, and quality of life.
  • Address and report any concerns related to abuse, neglect, or violations of residents' rights.
  • All Other Duties as Assigned.
