The Social Worker is responsible for participating in an interdisciplinary team approach to serve the needs of the Center population. They provide therapeutic intervention to help residents cope with the social and psychological aspects of their illnesses, impairments, or disabilities, as well as their feelings about institutionalization and separation from family, community, and loss. The role involves maintaining written documentation in the resident medical record, performing initial psychosocial needs assessments, and developing the social service component of the resident's plan of care. This includes identifying specific problems, goals, and approaches, and participating in interdisciplinary care planning conferences. The Social Worker ensures resident and/or family input into the care planning process, acts as a liaison between residents, families, outside agencies, and the Center Administrator, and conducts family meetings as needed. Facilitating discharge planning, ensuring customer safety, coordinating admissions, developing relationships with referral sources, verifying insurance coverage, and conducting facility tours are also key responsibilities.
