About The Position

The Integrated Care Team Social Worker provides timely coordination of quality healthcare services to meet individual’s specific healthcare needs in a cost-effective manner; this involves adaptability to meet community members at health centers, in their home or other community sites. Provides and coordinates care with patients and involved people for an appropriate plan that meets the medical, rehabilitative and financial needs of the individual. Works to provide community members support for social determinants of health barriers. Uses motivational interviewing techniques, advocacy skills and knowledge of mental health and community resources to overcome barriers, offers assistance from community organizations, other team members, and care coordination as appropriate. Applies social work methods to provide emotional and mental health support to individuals.

Requirements

  • Current Maine licensure in social work.
  • Bachelors degree in social work from an accredited institution.
  • AHA Basic Life Support.
  • Valid Drivers License.
  • Minimum two years of experience in the field of license.
  • Demonstrated knowledge of health, social service, and funding sources.
  • Within the scope of practice and license, must have the ability to work independently with limited supervision, capability to make independent decisions regarding counseling, education or referral of patients.
  • Maintains confidentiality.
  • Reliable personal vehicle or mode of transportation.
  • Emotional maturity, resilience and stability.
  • Knowledge of best practice standards and ethics.
  • Ability to multitask, prioritize and organize.
  • Ability to form collaborative, proactive and patient-focused relationships to focus, facilitate and maximize client healthcare outcomes.
  • Demonstrated communication and leadership skills with healthcare team members to facilitate maximal client outcomes.
  • Able to sit, stand, walk for 8 hour shifts.
  • Able to carry 20 pounds.
  • Effectively communicates verbally and written.

Nice To Haves

  • Experience in community and hospital social work and discharge planning preferred.
  • Experience counseling individuals and groups with behavioral, emotional and behavioral disorders.
  • Motivational interviewing experience preferred.
