Social Worker - Per Diem
About The Position
Directs and coordinates the integration of Social Services functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities; provides direct care to selected patients as appropriate; ensures compliance with quality patient care and regulatory standards. Facilitates and coordinates hospital-wide discharge planning and support programs to ensure compliance with Joint Commission and other health care accrediting standards in order to enhance the quality of patient care consistent with hospital policies for the adult and geriatric patients, 18 years and over.
Requirements
- Master level Social Worker degree
- Current certification issued in the state of facility location
- Minimum of three (3) years acute care experience to include experience in discharge planning procedures
- Medicare/Medicaid guidelines/knowledge of community resources
Nice To Haves
- Preferred Master level social worker/advanced clinical practitioner
- LICSW or LGSW preferred
