About The Position

Social Worker Join One of Northeast Florida's Premier Senior Care Communities River Garden Hebrew Home, an award-winning, not-for-profit, five-star nursing and rehabilitation facility located in Jacksonville's beautiful Mandarin area, is seeking an experienced and dynamic Social Worker to join our Social Services Department. As a Social Worker in our facility, you'll play a key role in helping residents and their families navigate care transitions, emotional challenges, and support services. This is a unique opportunity to make a meaningful difference in the lives of both short-term rehab patients and those requiring long-term support.

Requirements

• A caring, empathetic personality with a genuine desire to help others
• Strong communication and interpersonal skills
• Ability to handle sensitive situations with professionalism and compassion
• Bachelor's or Master's degree in Social Work (BSW or MSW)

Nice To Haves

• 2-3 years experience in a nursing home or healthcare setting preferred

Responsibilities

• Provide emotional support and counseling to residents and families
• Coordinate care plans in collaboration with our interdisciplinary team
• Advocate for resident needs and preferences
• Ensure documentation complies with regulatory standards
