The Social Worker (MSW) provides non-clinical biopsychosocial assessments and interventions on inpatient floors and in the outpatient emergency department. The role includes crisis intervention, patient and family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other identified resources. The Social Worker assists with post-acute care placement for discharges, particularly for patients with post-discharge behavioral health, assistance, or resource needs. The role is grounded in patient- and family-centered care, personal self-determination, prioritization, and compassionate problem-solving. The position serves intensive care units, Trauma, Oncology, NICU, Transplant, and the ED, and responds to physician and other non-discharge-planning social work consults throughout the facility. Working collaboratively with RN case managers, nurses, physicians, insurers, and other staff, the Social Worker identifies patient needs and barriers to discharge and recovery; assesses high-risk patients and develops discharge plans; coordinates care-team and family meetings; supports patients and families in decision-making; and provides consultation, education, counseling, advocacy, referrals, and emotional support for psychosocial needs and difficult or emotionally charged situations.
