About The Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. The Care Coordination, Advocacy, Treatment, and Connections to Housing (CATCH) Social Work Program Coordinator provides professional, clinical, and administrative case management for the CATCH Program in the Social Work Service care line. The (CATCH) Social Work Program Coordinator provides Social Work services on an outreach basis, specifically targeting Veterans experiencing homelessness who have high utilization of VANCHCS and community emergency departments.

  • Social workers must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7401(3).
  • Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.
  • A doctoral degree in social work may not be substituted for the master's degree in social work.
  • Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

  • One year of experience equivalent to the GS-11 grade level.
  • Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency.
  • Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
  • Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
  • Ability to supervise multidisciplinary staff assigned to the program.
  • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Ability to provide training, orientation, and guidance within clinical practice.
  • Position will require the operation of a motor vehicle.
  • Valid unrestricted drivers license required.

Responsibilities

  • Collaborates closely with VA Homeless Programs including, but not limited to, Grant and Per Diem Programs (GPO), Healthcare for Homeless Veterans (HCHV), HUD-VASH, as well as other departments at the VA Northern California Healthcare System (VANCHCS) and community homeless providers across the spectrum on the local, state, and federal levels.
  • Assists in developing individual or group goals and objectives as well as monitoring and coordinating the VANCHCS CATCH Program, follows VA-OIG, JCAHO and state standards in the performance of duties.
  • Collects and submits participant data into CPRS, REDCap, HOMES, and other VA auditing procedures on a regular basis.
  • The (CATCH) Social Work Program Coordinator independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs.
  • Possess advanced knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society, and culture.
  • Demonstrates a mastery of theories and treatment modalities used with this complex population, an ability to diagnose and incorporate differential diagnoses and treatments, and an ability to work with patients and families who are experiencing basic emotional, medical, and social problems utilizing advanced and specialized interventions and treatment modalities including individual, group, and family counseling.
  • Works collaboratively with the team and collateral staff to develop the CATCH Program and outreach to VA and non-VA programs in the Emergency Department (ED), Walk-In Clinic, Homeless Patient Aligned Care Team (HPACT), Housing and Urban Development-Veterans Affairs Supported Housing (HUD-VASH), Mental Health, Substance Use/Dependence, Social Work, and other relevant service lines for increased collaboration and coordination of services.
