About The Position

The Pediatric Palliative Care Social Worker, reporting to the Palliative Care Clinical Manager, is responsible for providing ongoing psychosocial assessments and advocating for interventions that alleviate burden and facilitate coping for assigned pediatric palliative care patients and their families. This role involves participation in family support, teaching, counseling, education, and advanced care planning in collaboration with the pediatric palliative care interdisciplinary team.

Requirements

  • Master’s Degree in Social Work
  • LICSW or LCSW required
  • Demonstrated experience working with children and families in clinical capacity
  • Demonstrated experience working with life-threatening conditions and chronic disease
  • Strong verbal and written communication skills with patients, caretakers and providers
  • Demonstrated ability to work independently with minimal supervision
  • Demonstrated Ability to work as a member of a multidisciplinary team
  • Demonstrated organizational skills
  • Time management skills

Nice To Haves

  • One year work experience in hospice and/or palliative care
  • Experience in the use of electronic medical record
  • Fluency in Spanish and/ or Portuguese
