About The Position

Humber River Health is seeking a Palliative Care Social Worker to provide specialized psychosocial support to patients with life-limiting illnesses and their families across hospital programs. This role involves working within an interdisciplinary team to conduct comprehensive assessments, support goals of care and advance care planning, offer counseling, grief and bereavement support, and facilitate access to community resources. The Social Worker will play a key role in enhancing quality of life, supporting patient and family-centered decision-making, and ensuring timely access to appropriate palliative care services. Requirements

  • MSW obtained from an accredited school of Social Work required.
  • Current membership with OASW and OCCSWSSW, or eligibility to become a member required.
  • Computer skills required; ability to manage electronic recording, workload and meet submission timelines.
  • Knowledge of palliative care principles, symptom management, end of life care, advanced cade planning, goals of care conversations.
  • Experience supporting patients and families through grief, loss, bereavement and complex decision – making.
  • Strong knowledge of community palliative care resources, hospice services, Ontario health at Home and care transition pathways.
  • Demonstrated organizational skills to set clinical and non-clinical priorities in a fast-paced environment.
  • Demonstrated ability to work independently and in a team environment.
  • Demonstrated excellence in interpersonal skills.
  • Excellent communication skills (oral and written) facilitating effective communication in case reporting and assessments, with patients, families, peers, other healthcare team members and volunteers.
  • Excellent decision-making skills.
  • Demonstrates cross cultural awareness and sensitivity.
  • Excellent attendance and discipline free record required.

Nice To Haves

  • Specialized training in palliative care (i.e., LEAP, CAPCE), advanced care planning, goals of care discussions, grief and bereavement counselling is an asset.
  • Demonstrated experience in palliative care, geriatrics, complex chronic disease management or related healthcare settings preferred.
  • Experience in the acute and/or sub-acute sector system, patient flow and discharge planning is an asset and preferred.

Responsibilities

  • Provide social work support to palliative patients and families across multiple clinical programs, including acute medicine, surgery, critical care, and geriatric services.
  • Support identification of patients with life-limiting illness, conduct comprehensive psychosocial assessments for patients with life-limiting illness and their families/caregivers.
  • Support goals of care discussions, advance care planning and decision making in collaboration with the interdisciplinary team.
  • Act as liaison and assist patients and staff with community resources.
  • Coordinate complex discharge planning and patient flow.
  • Act as a resource to patients, families and healthcare providers regarding palliative care services, community resources and system navigation.
  • Participate in interdisciplinary rounds and contribute to individualized palliative care plans.
  • Works as a member of the health care team.
  • Flexibility in caseload and program assignment.
  • Able to manage and prioritize patient caseload in a busy acute care setting.
  • Participates in reflective practice as per College standards.
  • Clinical supervision of social work students.
  • Provides cross-coverage of Social Work Services for vacation/staffing relief.
  • Cross coverage may also include coverage at the Reactivation Care Centres (RCC) located at the Finch Site and Church Sites.
